The <abbr>UC<\/abbr> team identified the likely physical cause to be an electrostatic charge and also provided recommendations on how <abbr title=\"University of Hawaii\">糖心Vlog官方<\/abbr> and universities and research facilities across the country can improve laboratory safety practices. <\/p>\n<ul>\n<li><a href=\"https:\/\/cls.ucla.edu\/images\/document\/Report%201%20糖心Vlog官方.pdf\">Report 1: Technical Analysis of Accident<\/a> (<small>PDF<\/small>)<\/li>\n<li><a href=\"http:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2016\/07\/Report-2-University-of-Hawaii.pdf\">Report 2: Recommendations for Improvements in <abbr>糖心Vlog官方<\/abbr> Laboratory Safety Programs<\/a> (<small><abbr>PDF<\/abbr><\/small>)<\/li>\n<\/ul>\n<p><abbr>糖心Vlog官方<\/abbr> retained the <abbr>UC<\/abbr> Center for Laboratory Safety for the investigation because it is a national leader in developing evidence-based best practices and facilitating implementation and optimization of laboratory safety practices. <\/p>\n<figure id=\"attachment_44007\" aria-describedby=\"caption-attachment-44007\" style=\"width: 260px\" class=\"wp-caption alignright\"><img loading=\"lazy\" decoding=\"async\" src=\"http:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2016\/03\/manoa-post-building-260x146.jpg\" alt=\"\" width=\"260\" height=\"146\" class=\"size-medium wp-image-44007\" srcset=\"https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2016\/03\/manoa-post-building-260x146.jpg 260w, https:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2016\/03\/manoa-post-building.jpg 620w\" sizes=\"auto, (max-width: 260px) 100vw, 260px\" \/><figcaption id=\"caption-attachment-44007\" class=\"wp-caption-text\">University of <span aria-label=\"Hawaii\">Hawai&#699;i<\/span> at M&#257;noa Pacific Ocean Science and Technology Building<\/figcaption><\/figure>\n<p>&ldquo;The University of <span aria-label=\"Hawaii\">Hawai&#699;i<\/span> thanks the <abbr>UC<\/abbr> team for its thorough investigation,&rdquo; said <abbr>糖心Vlog官方<\/abbr> M&#257;noa Vice Chancellor for Research <a href=\"https:\/\/manoa.hawaii.edu\/ovcr\/bio-bruno.html\"><strong>Michael Bruno<\/strong><\/a>. &ldquo;The lab safety recommendations provide valuable guidance in our commitment to a culture of safety in each and every research and teaching laboratory on our campus.&rdquo; <\/p>\n<p><abbr>糖心Vlog官方<\/abbr> M&#257;noa has already taken multiple steps to improve lab safety including the creation of a Chemical and Physical Hazards Committee made up of representatives from all of the schools and colleges that have research and teaching laboratories. This is one of the key recommendations by the <abbr>UC<\/abbr> Lab Safety Team. The committee will work with other safety-related committees already in existence to identify and implement protocols and processes to further strengthen the safety of <abbr>糖心Vlog官方<\/abbr> laboratories. <\/p>\n<p>&ldquo;Our goal now is to become a national leader in laboratory safety.&rdquo; said Bruno.  &ldquo;It\u2019s about enhancing our safety culture and standard operating procedures where risks are constantly being identified and minimized. Everybody has a role to play.&rdquo; <\/p>\n<p>The accident happened in a <a href=\"http:\/\/www.hnei.hawaii.edu\/\"><span aria-label=\"Hawaii\">Hawai&#699;i<\/span> Natural Energy Institute<\/a> (HNEI) laboratory and seriously injured a researcher. Following the accident, the university suspended activities at <abbr>HNEI<\/abbr> laboratories working with high-pressure gases, flammable or explosive materials and other high-pressure work. The labs are still off line and are in the process of completing safety reviews of all activities and equipment.  <\/p>\n<ul>\n<li><a href=\"http:\/\/www.hawaii.edu\/news\/wp-content\/uploads\/2016\/07\/160630-HNEI-Lab-Safety-Walkthrough.pdf\"><abbr>HNEI<\/abbr> Lab Safety Walkthrough<\/a> (<small>PDF<\/small>)<\/li>\n<\/ul>\n<p>The university is conducting regular safety peer-reviews among researchers and laboratory staff.
